Social Security Card Application

This federal form is used to apply for a new or replacement Social Security card, or to request a correction to your Social Security record—such as a legal name change. You must print the form, fill it out completely, and submit it in person or by mail with original supporting documents. The Social Security Administration does not accept photocopies or notarized copies of your ID or legal documents.
